Patients are told that while their information and opinions in the survey is important to their medical provider there is no requirement that they complete this survey.  They may choose to participate or not, but the more people who respond, the greater the ability to improve the quality of medical care provided. 

Also, if patients choose not to participate in the survey there is no effect on the healthcare they receive since the medical practice sees only survey results
